Reversal of Direct Oral Anticoagulants: Guidance from the Anticoagulation Forum
Join lead authors Adam Cuker, MD and Allison Burnett, PharmD for a discussion of the recently published: Reversal of direct oral anticoagulants: Guidance from the Anticoagulation Forum.

We are pleased to announce FREE Open Access to this document, published in the American Journal of Hematology, April 2019.

To review the guidance document, visit: https://onlinelibrary.wiley.com/doi/full/10.1002/ajh.25475

The goal of this document is to provide comprehensive guidance from the Anticoagulation Forum regarding use of DOAC reversal agents. We discuss indications for reversal, provide guidance on how the individual reversal agents should be administered, and offer suggestions for stewardship at the health system level.

As with previous Anticoagulation Forum guidance documents, we prioritized a set of key questions regarding DOAC reversal through discussion and consensus among the authors. For each question, a summary of the evidence is provided, followed by guidance representing unanimous consensus of the authors.

Adam Cuker, MD, MS, is Assistant Professor of Medicine and of Pathology and Laboratory Medicine at the University of Pennsylvania Perelman School of Medicine in Philadelphia. He also serves as Director of the Penn Comprehensive Hemophilia and Thrombosis Program and Associate Director of the PennCHOP Blood Center for Patient Care and Discovery.

Dr. Burnett is Director of the Inpatient Anticoagulation Management Service and Coordinator, Inpatient Antithrombosis Stewardship at the University of New Mexico Hospital and an Assistant Clinical Professor at the University of New Mexico College of Pharmacy.
