High-functioning clinical care teams routinely use standardized pathways and tools to improve quality. Most of the pathways and tools that have been developed to date are for in-person care. A multi-disciplinary team from Health Management Associates (HMA) developed pathways for four common conditions—asthma, diabetes, hypertension and depression. This presentation will provide an overview of the current and predicted telehealth landscape, discuss the challenges and benefits from developing pathways for care delivered through telehealth, and demonstrate portions of the pathways.
