Join us as we share results from Stronger Together; a program designed to support patients at home pre and post joint replacement surgery during COVID-19. Patients participated in a week-by-week evidence-based virtual care program designed to improve health literacy, confidence and outcomes through a combination of peer-to-peer social support, secure at-home remote monitoring of key vitals and virtual one-on-one coaching from nurses and experts in a private social health network.
