The Right Care at the Right Time: Incorporating Nutrition Care for Diabetes into Primary Care
Medical Nutrition Therapy (MNT) reduces blood sugars throughout the continuum of diabetes care, yet only 5% of Medicare beneficiaries with diabetes, and only slightly more persons with private insurance coverage, are receiving cost effective nutrition interventions. Primary care practices can improve outcomes through evidence-based nutrition interventions provided by registered dietitian nutritionists (RDNs) in populations with diabetes. Learn how to integrate and/or coordinate that care, including the revenue streams available for these services. This presentation will describe how Medical Nutrition Therapy improves important outcomes in populations with diabetes; discuss Integrating nutrition care for diabetes and related chronic conditions into various types of primary care practices and provide an estimate of the potential revenue streams to pay for services provided by RDNs in primary care.
