Reconstruction of Complex Defects and the Interaction of Hard and Soft Tissue
Complex defects always mean horizontal as vertical compromised tissue and it needs to be repair work done, not only on the hard tissue much more the soft tissue as well.
A lot of techniques can be successfully performed, we try to show and explain some of them. If possible, we try to perform the augmentation together with the implant placement, where as we will work on the gingiva during the 2nd stage surgery with roll flaps, modified rotation flaps and mostly modified free gingival grafts.
There will be solutions with a tenting approach and with Re - Oss 3 D cages, customized on the DICOM datas of the patients defect, performed by an DVT or CT-Scan.
In order to accept as little compromises as possible, the handling of the soft tissue is most important. The tension free closure of the split flaps with anchor sutures is supported by sticky bone, a mixture of growth factors and bone chips, the cages or bone plates. Missing more than one element in the esthetic zone will lead to a pontic design of the fixed restoration, in all that situations it is highly recommended that the dental technician is able to understand the prosthetics have to fit into the tissue, so as the treating doctor needs to know a lot about the work in the dental laboratory.
