I have a question, we have quite a few surviviorship events but have a low turn out for participants. How can I get better buy in and spread the word to get more participants in things like support groups, education events, etc.

Focus has been on workflow for providing survivorship care plans and ways to integrate care plans into the EHR. We have a number of classes that are offered to survivors but this takes quite a bit of work and resources and turn out is not huge. we likely need to investigate what patients need...do they even want these classes? What format would be best for them to receive survivorship information? We have NP led survivorship visits but these are typically one time after completion of treatment. I would like to focus on patient reported outcomes.

We plan to survey primary care providers about their experience with care plans and if they find them valuable, are they reading them, using the content, etc. Has anyone else done such a survey?
