Many Medicare telehealth services are billed using the same codes as in-person services,with a modifier to indicate that they were provided via telehealth. If the code billed for atelehealth service is included in the list of codes eligible for the flat primary care visit fee(e.g., an evaluation and management (E/M) visit), is billed by one of the Primary Care Firstpractice’s clinicians, and meets the Medicare telehealth coverage requirements (e.g.originating site requirements), the flat primary care visit fee applies. Further guidanceregarding requirements and potential waivers will be issued at a later date.

No, the requirement to provide health services to a minimum of 125 attributed Medicare beneficiaries does not apply to practices participating in the SIP only option. It does apply to hybrid practices participating in PCF general and SIP.
