One of our FSP providers in LA County developed the Determinants of Care to help FSP teams determine readiness to step down. It is completed at that provider site by teams in their regular team meetings once a month. We didn't implement it countywide due to a firm stance from our management on not adding to provider burden with increased paperwork and reporting. Contact me if you are interested in hearing more if it might help anyone. Aurelle or Zach have my contact info.
