I have spoke to someone in AHS who has submitted the original form (can't remember the number) and they were then asked to fill out a more detailed form with what religion, if you had previous vaccines etc.

Hahahahahaha. No one ever saves the social workers. I have just been biding my time. I am here to help once I get my feet on the ground if I can - but I can start with my own tomorrow. I have nothing to lose - lol. They have made it incredibly difficult to just do my job. It’s not about the money - its about the ethics and moral bankruptcy we are facing from all employers.
