Do we need to be from a state department to apply? Or as a nonprofit can we apply and include confirmed partnerships with state departments, PCAs and DVCs?

Are those tools available generally - even if we don't get this grant? we would be interested in both the policy landscape tool and the referral tracking tool for other work we are doing. Thanks for any sharing you can do!

However, if there are 5 CHCs and DVPs in the state or territory, then the expectation would be 5 CHC and DVP parterships. Thank you!
