@Vlai, yes, if your TIN has already been validated under previous General Distribution phases, you must reenter the portal by 11/6 @ 11:59PM EST and will have until 11/27 @ 11:59PM EST, to complete and submit an application
I applied for and received funding from phase 2. I applied and received a reply that my application for phase 3 was accepted, but have been unable to determine the status of that application. Is this indicative of a problem?

If due to a CHOW, the targeted Infection Control distributions were returned by the prior owner. Will the Phase 3 application help us to receive those funds?

I submitted Phase II under one application with our management entity EIN and listed the 22 other EIN's. It was valiated but no funding has been seen. Then in Phase III I applied with the 22 individual EIN's. Will this cause a problem with getting approval and/or funding?

Phase 3 of the General Distribution will take into account documentation of financial impact of COVID-19, as reported by applicants. The payment methodology will ensure a provider has received 2% of annual revenue from patient care either as part of the previous phases of the General Distribution or under a Phase 3 payment. Phase 3 may also take into account a provider’s change in operating revenues from patient care, minus their operating expenses from patient care. Phase 3 payment will also take into account funds received and kept under prior General and Targeted Distributions. While HHS has made payments on a rolling basis under the previous general distributions, Phase 3 final payment amounts for applicants who have already received payments equaling 2% of annual patient care revenue will be determined once all applications have been received and reviewed.

Providers who have previously received a payment under Phase 1 or Phase 2 of the General Distribution are eligible to apply for a payment even if they have previously received a disbursement of 2% of annual revenue from patient care. Providers who have not previously received a General Distribution payment, or an amount that is less than 2% of patient care revenue, may also apply for funds if they meet the above eligibility criteria.

This is Mike McElroy from Highland Assisted Living and Memory Care in Jackson Michigan. I applied for Phase 2 Provider Relief funding back in September and I never heard back about verification for my TIN. I have subseguently applied for Phase 3 a few days ago. It says my TIN is being verified again. How can I follow up on my Phase 2 application.
Thank you for taking the time to discuss! The HHS FAQs state that TIN validation AND financial information is due by 11/6 for any entity that either applied for Phase 2 funding OR received general distribution phase 1 funding. The HHS Provider Relief hotline is providing the guidance that because the TIN validation feature was introduced in Phase 2, the 11/6 deadline only applies to entities who applied for phase 2 and already underwent the TIN validation process. Could you please provide some clarification as to which of these two pieces of guidance is accurate?

If our AL and SNF are under the same TIN, do we have to submit sperate applications or just one?
We received monies in Phase 2 that were deposited into a bank account. However, we have no link to the account. UHC tells us they cant give us any information besides the fact that the account ends in 9. How can we verify an account number?
You can't go further in the application until your TIN is verified
What if we do not have an NPI #? WE at St. Katharine's are still awaiting TIN approval.

We have been waiting on our Tins from phase 2 to get validated for about 2 months now. The online portal will NOT allow us to submit for phase 3 while we are awaiting Tin validation.

We applied for phase 1 as well as phase 2, but both applications did not move beyond TIN verification phase. How do we make sure that we get through TIN verification. I believe out TIN is not on curated list.
There's a problem in getting past the page to apply for Phase 3 because of not being able to attest to Phase 2 money being deposited into the account. Still waiting on Phase 2 funds. My questions are:1- If we applied for Phase 2 and were approved, is our TIN still approved for Phase 3 for the Oct 7 deadline or do we have to re-submit for Phase 3 approval?2- If it is still approved, do we have until the end of Nov to complete the Phase 3 application and upload our financials?3- IF, our TIN is not approved for Phase 3, and we can’t get past this screen, will we miss the Nov 7 deadline?

When can we expect our Phase 3 funding?
Has there been any consideration for a smaller percentage of AL providers who operate a start up development for Assisted Living during COVID. Newly built developments in the first 18-24 months lose money but revenue increases. So the impact on revenue is not fairly stated by the application process or method of determining loss. Start up occupancy/revenue, negatively impacted but revenues increase. Any way to explain a special circumstance or is that a possibility?
Oklahoma here, I run a non-profit SNF and AL. I got phase 1 and 2 on the SNF, but nothing on the AL. How do I make sure we are listed on your list as both SNF and AL? Thank you.

Both phase 2 and Phase 3 only asked for TIN numbers and said it was complete. If I try to go further it wants another TIN.
We have a community that operates private pay IL, AL and MC as well as SNF that has medicare. We received targeted distributions for the SNF as part of phase I. We submitted our phase II application on 2% of 2019 tax return revenue (including the SNF) and received a payment; however it was reduced by the targeted SNF payment previously received. It was our understanding that targeted distributions would not impact the amount for Phase 2?

I am trying to ask a question we have some new communities that were start ups in 2019 so the Q1 and Q2 2019 compared to Q1 and Q2 of 2020 will not support the lost revenue of move ins and additional Covid expenses

I went into validate my TIN how long does it take to know if it is validated? Right now it is showing Validate TIN - Processing.
We submitted TIN's for phase 2 they stayed in Validation in Process. when I called about phase 3 I was told to remove the TIN's and resubmit for phase 3. No payment for phase 2 was received. No application was able to be submitted at all for phase 2 because TIN's were never verified.

Our business changed from 2019 to 2020 and revenue is not down, but expenses are much higher. It is a flawed theory that 2019 should relate to 2020
Being unable to market and consistently lower census is really hurting here in Q3

I just spoke to United Health /Argentum and they said that if your Tin says it's validated from Phase 2 then all you have to do is Submit the Revenues & Expenses which will be a slightly different application from Phase 2 because it asks for 2020 and 2019 Quarterly revenues/expenses etc. Then you should get the confirmation email
I was told when I called the help line that the Phase 3 applications won't be awarded/approved until after the deadline of November 6. As long as I have the email saying that my application was received, we are good and just need to wait.
any idea of when the funds for phase 3 will be awarded? Released?
Will the income statements be sufficient to support/back up the revenues and expenses that we are sending in?
