Yes, the idea of a direct contact to have at the ready as the go to that links patients directly with resources. It’s very difficult for providers to take the lead in that. We can screen, but we need the village to take the baton. Case managers would be fantastic!

Community Health Workers, System Navigators, and Peer advocates can assist and would provide non federal funding which can be matched by federal entitlement dollars (Medicaid) which would increase more funding for support in the work.

We do ACEs screening embedded into our CANS-mental health assessments at our school based FQHC and community clinics… and I found that the conversations with families were important place to practice being with vs. fixing.. and building alliances with families who are in so much pain and naming these layers of pain—but I also find that all of us are doing many screens that also often result in “referrals to nowhere” or families who navigate too many referrals from so many of us who are not connected. I love that some counties are moving toward “One family, one care plan” so we are not overwhelming families and also doing linkages and not simply referrals to voicemails or waitlists..

Agree CHW's, System Navigators, and peer advocates is a great alternatives as they can be part of the referring clinic. Often times when a report is done to CPS doesn't necessarily mean that family will get connected to all the resources needed.

there is typically a large stigma for mental health and social work. So physicians get releases for medical, but a referral to mental health or social work is more likely to be refused or not followed up upon /responded to by families
